Grange Cottage is a dynamic, progressive and innovative Residential Care Home based in Sutton in the heart of busy South West London, which provides person centred care for people living with dementia. Dementia is a syndrome (a group of related symptoms) associated with an ongoing decline of brain function.
This may include problems with: impairment of memory and cognitive function such as memory; recalling; thinking speed; mental sharpness and understanding; judgement; mood movement, as well as difficulties carrying out daily activities. There are many different causes of dementia. This affects all aspects of personality, behaviour and conduct. The care service we provide is very flexible and is based on awareness that each individual has the right to be loved, listened to, understood and respected.
On admission all residents undergo a period of comprehensive assessment of needs between four to six weeks. Following this period, comprehensive care plan will be implemented, depending on identify needs of the resident. Monthly review of the care plan is carried out jointly with the Health Care Professionals. Relatives are encouraged to participate in the evaluation process.
